Registration Instructions
Registration Process, in 6 steps, due by August 1, 2011*
1. Go to www.usahockey.com and register. You will receive a registration confirmation with a number. You must have this number to proceed.

2. Read and Review the following forms.
Financial Responsibility Agreement Player/Parent Contract
3. Go to the online registration site here and complete the information required. You MUST complete steps 1 & 2 before doing this. If you are new to the club and are in Grades 5 - 12, be sure to fill out and return the Warm-Up suit information on the online registration form.
5. Write a check payable to Penncrest Ice Hockey Club according to the payment schedule on the Financial Responsibility Agreement.
6. Mail the check to:

No player will be permitted to skate in any practices or games until all forms are submitted and all fees are current.
