Registration Instructions
Registration Process, in 6 easy steps, due by July 1, 2008*

Log on to www.pencrestlions.com, select the Ice Hockey Site (listen to the ROAR!)
Select “Registration”
Complete ALL registration forms listed below (ok, this step isn’t so easy…).
Write a check payable to Penncrest Ice Hockey Club for $500 (high school) or $400 (middle school) - (ok, neither is this step, but hey, it’s for the kids…).
Fill out Personal Information Form.
Mail the check and all forms to:

Registration Forms
· 2008-09 Penncrest Ice Hockey Club Personal Information Form
This form provides basic player information. Please provide all necessary information. THIS MUST BE RETURNED WITH A DEPOSIT BY JULY 1. We need to order warm-ups and workout gear for HS and MS players ASAP.
· 2008-09 Penncrest Ice Hockey Club Uniform Order Form
Complete this form if jerseys or shells are required. Please provide all necessary information.
· USA Hockey Consent to Treat Form
This form will allow your child to be treated if injured during a game or practice. Please complete both sides.
· Code of Conduct
This form ensures that the parents and players are fully aware of all club rules and the punishment for any rules violations. The signature of the player and parent is required.
· Financial Responsibility Agreement
This form is a promise to pay club fees by the required dates.
· USA Hockey Online Registration Instructions
Every player is required to register online with USA Hockey. Follow enclosed instructions.
Important Notice:
No player will be permitted to skate in any practices or games until all forms are submitted and all fees are paid.
