High School Players
Physical Exam and Concussion Testing
Like any other Penncrest student-athlete, ice hockey players are required to get a physical examination. Also, hockey players will be included in the baseline concussion testing. This will make participation safer and complies with the Safety in Youth Sports (HB 200) Act passed by the Commonwealth of PA.
You are free to get your physical done anywhere but that form must be completed. The physical form is located here. Simply click on the link and download the form. You will need to complete Sections 1 - 4. Your doctor will complete Section 5. Please bring the entire packet with you to your physical.
Baseline concussion testing will be held at Penncrest High School and conducted this coming fall.
